What's everyones view on this? Should it be set to allow all incoming connections(default) or block all incoming connections? or how should it be set up and is it effective?. I currently have a d-link router, and I connect wirelessly.

Use the 'Set access for specific services and applications' option. When an application wants to receive a connection it will prompt you to allow it. That will ensure that you don't allow everything free reign. In most cases you'll find there's only a couple of applications which require incoming access.

If you want to be paranoid about incoming and outgoing connections, get Little Snitch. The leopard beta is fantastic.
